Safe Discharge Home Questions Families Should Ask

A hospital discharge can be medically appropriate and still be difficult at home. That is the gap families need to pay attention to.

Hospitals are focused on whether a person still needs hospital-level care. Families are focused on whether the person can get into the house, take medications correctly, shower safely, eat, sleep, avoid falls, and be supervised if confusion or weakness shows up.

Those are related questions, but they are not the same question. A person can be stable enough to leave and still need a stronger home plan.

Before discharge, ask what has to be true at home that first night. Is equipment in place? Who helps with transfers? Who manages medications? Can the person be alone? What symptoms require a call or return to care?

Safe discharge should mean more than leaving the building. It should mean the next 24 hours have a real plan.
