Rehab Discharge Home Care Questions Families Should Ask
Rehab discharge can feel like a victory, and often it is. But leaving rehab also shifts responsibility back to the home.
Families should ask what the person can actually do safely. Can they transfer from bed to chair? Use the bathroom? Shower? Manage stairs? Prepare food? Remember medications? Be alone for several hours?
Therapy progress matters, but a therapy milestone is not the same as a home routine. A person may walk farther in therapy than they can manage safely in a cluttered hallway or bathroom at home.
Before discharge, ask what equipment must be ready and what tasks still require another person. Home health may help with skilled therapy or nursing. Home care may help with bathing, meals, supervision, and daily support.
The first week home deserves a real plan. Schedule help around the riskiest moments, not just the easiest hours.
